I was pretty far along after doing the crest pieces. Decided to go back and visit the Gnoll. Scared him and he gave me another crest piece.

No! He just (or voice says) says he already gave you what you want.

I think the coding is straightforward and it has the scare option regardless of prior acquisition of the seal piece.

I think what's going on here is that you probably got the crest the first time via tobacco as opposed to the spell--we need to add a separate check to the code to make sure you haven't done either action before.  Anyway, we've logged the bug and will fix it for the coming update.  Thanks!
